How we work with referrers

At JDS Care Provider, we provide a structured and reliable intake process designed to ensure safe, appropriate, and well-matched support delivery under the NDIS.

We work closely with Support Coordinators, Plan Managers, and families to ensure a seamless transition into services across Port Hedland, South Hedland, and the wider Pilbara region.

Referral pathway

How to Refer a Participant

Transparency, safety, and the right fit at every stage.

Initial Referral & Review

Submit a referral via our intake form on Google Forms. All enquiries are reviewed within 24–48 hours.

At this stage, we assess functional needs, support requirements, and alignment with the participant’s current plan under the NDIS.

Consultation & Service Alignment

We complete a structured review of goals, preferences, and support requirements.

Where required, a consultation is arranged to confirm suitability and ensure our services are aligned with the participant’s needs and desired outcomes.

This step ensures clarity, safety, and a personalised support approach.

Service Agreement & Onboarding

Once suitability is confirmed, we proceed with formal onboarding.

  • Reviewing and signing a Service Agreement
  • Completing required onboarding documentation
  • Establishing a clear and transparent service framework

This ensures all supports are delivered within a compliant, structured, and accountable model.

After onboarding

Participant Matching & Service Commencement

Following onboarding, we match the participant with a consistent support professional suited to their needs, preferences, and goals.

Where appropriate, an introduction is arranged prior to commencement to ensure comfort, continuity, and a smooth transition into services.

Step 1 — Refer Most referrals take about 10–15 minutes once details are prepared.
